e-mail requests for records or record checks to:
policereports@clydeohio.org . These requests will be
processed in a timely manner. At this time, we are unable to
e-mail the Ohio OH-1 accident report form. Requests for accident
reports can be made in person or in writing to the Clyde Police
Department 222 N. Main St. Clyde, Ohio 43410, written requests
must include a self addressed stamped envelope. The police
department does not charge for basic police reports, including
accident reports, please do not send money for these reports.
Requests for photographs can be made to the above e-mail
address. The Clyde Police Department has recently started using
digital photography for most incidents. Digital photographs will
be sent by e-mail attachment. For standard film photographs, the
requestor is required to pay for processing before the
photographs will be mailed or given out.
Effective 05/01/2005 the Clyde Police Department no longer faxes
record checks or Clyde Police Department reports, including OH-1
accident reports.

Civilian Finger Printing
The following is the fee for those individuals that have lived
within the State of Ohio for more than five (5) years.
(1)
Residents of the City of Clyde: Federal (currently
$24.00) and State Fee (currently $22.00) –plus- $5.00
(2)
Employment within the City of Clyde: Federal
(currently $24.00) and State Fee
(currently $22.00)
–plus- $5.00
(3)
Employment with Clyde-Green Springs School System:
Federal (currently
$24.00) and State Fee
(currently $22.00) –plus- $5.00
(4) For non-residents of The City of Clyde, or those
individuals not employed within the City or employed by the
Clyde-Green Springs School System, shall be charged, in addition
to the Federal (currently $24.00) and State Fees (currently $22.00), a fee of forty-five dollars
($45.00) for one or both sets submitted.
Fingerprinting service available 24 hours daily. Appointments
not required, however you may want to call ahead to see if staff
